BSSI USPS Form 1583

The United States Postal Service (USPS) introduced a new Form 1583, which must be completed by all clients of BSSI, a USPS Commercial Mail Receiving Agency (CMRA). The BSSI USPS Form 1583 is a must for compliance.

The BSSI USPS Form 1583:

  • Confirms your intention of having a Private Mailbox (virtual office) with BSSI.
  • Authorizes BSSI to manage your mail.
  • Documents the individuals authorized to use the address.
  • Prevents unauthorized interception of mail.
 
To maintain its operational status as a USPS agency, BSSI must collect the BSSI USPS Form 1583 from clients for each company account. Per USPS, “failure” to comply will subject the agency to withholding of mail from delivery until corrective action is taken.”
Clients must also provide two forms of identification for each individual associated with the company who receives or picks up mail.

  1. Download and complete the BSSI USPS Form 1583 using either an online notary service or sign the Form with a BSSI representative.
  • Multiple company accounts? Complete this Form for each Company.
  • Accepted types of IDs are listed on the Form.

2. Email BSSI your completed notarized Form 1583 and 2 forms of identification for every contact that handles mail by December 31, 2024.

Section 1

When your account was set up, you received a Go Live email. That date is when your PMB opened. Can’t that email? We can complete that field when you return the Form.

Your BSSI address is 3651 Lindell Rd Ste D, Las Vegas, NV 89103. Enter your mailbox number in section 2 B.

Section 3

Type of Service Requested is BUSINESS/ORGANIZATION

Section 4

The Applicant is the Primary Contact for the Account.

Section 5

The Authorized Individual is different from the Applicant. Only complete Sections 5, 1,0, and 11 if there is someone else picking up mail.

Section 6

If BSSI forwards your mail, enter the forwarding address. If you have a scan service, enter your email address.

Section 7

On this Form, your business address is NOT the BSSI address. Enter either your home address or your business’s physical location (if applicable).

Section 8

Refer to Section 8e for the accepted identification that can serve as a photo ID for the Applicant. U.S. State/Territory/Tribal Driver’s and Nondriver’s IDs can be used as either a Photo ID or an Address ID, but not for both.

Section 9

Refer to Section 9g for the accepted ID that can serve as the Applicant’s ADDRESS ID. U.S. State/Territory/Tribal Driver’s and Nondriver’s IDs can be used as either a Photo ID or an Address ID, but not for both.

Section 10

Only applies if there is an Authorized Individual (Section 5). Refer to Section 10e for the accepted identification to serve as photo ID for the Authorized Individual. A U.S. State/Territory/Tribal Driver can be used as either a photo ID or an Address ID, but not for both.

Section 11

Only applies if there is an Authorized Individual (Section 5). Refer to Section 11g for the accepted IDs that can serve as the Address ID for the Authorized Individual. A U.S. State/Territory/Tribal Driver can be used as either a photo ID or an Address ID, but not for both.

Section 12

Will the Authorized Individual in section 5 receive mail? Enter your name in this field. Are there other contacts at the company who receive mail? Please enter the names and separately email BSSI their two forms of ID, following the ID requirements listed earlier.
